MHS GENESIS Patient Portal is a great way to review your (1) health record, (2) send messages or documents to our Primary Care Staff, (3) request prescription refills, (4) fill out forms before your appointments, (5) access educational content, and (6) complete an eVisit.

All items must be completed and submitted to the clinic before an appointment can be scheduled with a provider. You can turn in the items in person or via the MHS GENESIS Patient Portal to an assigned Temecula clinic provider.
- Completely fill out and sign the following:
DD FORM 2807 (ensure all YES answers on the DD form 2807 are properly noted and explained), DD FORM 2808 , TBI and PTSD Checklist, DD FORM 2813 DoD Dental Exam, SHPE\SEPARATION COVER SHEET.
- You are NOT required to get the DD FORM 2813 DoD Dental Exam Signed if you are dental class 1 and have had a dental exam within the last year from the date of separation. Otherwise, you will need to schedule a dental appointment and get the DD form 2813 Signed.
- Once all forms are completed you will need to attach a copy of your Individual Medical Readiness (IMR) (HIV blood draw must be completed within 1 year of Separation date). IMR can be found on Marine Online for USMC, BUPERS Online for Navy, ASIMS for Air Force.
- Please attach a copy of your termination audiogram (an audiogram must be performed within 6 months prior to separation)
- For members with corrected vision, an eye exam must be completed within 2 years of separation, otherwise, exam must be within 5 years.
- Ensure all immunizations are complete and up to date.
- Females: Ensure if required all well women exams are up to date (mammograms, pap smear etc.)
- After submitting the packet, please allow 3-5 business days for review and for the provider to schedule your appointment.
- After submission and review, expect a 1 to 2 week turn around for an in-person appointment.

- Receive a Reenlistment Packet from your command and completely fill out and sign the packet.
- Ensure any readiness items are completed for the Individual Medical Readiness (IMR) (HIV blood draw must be completed within 1 year of medical appointment date). IMR can be found on Marine Online for USMC, BUPERS Online for Navy, ASIMS for Air Force.
- Once complete, submit the reenlistment packet to the clinic for review.
- After the packet has been reviewed you will be contacted to schedule an in-person appointment at the clinic.

All items must be completed and submitted before an appointment can be scheduled with a provider. ALL PHA’S ARE TO BE DONE THROUGH THE NAVAL HOSPITAL.
- Go to Sign in - EHA (health.mil) and fill out the “Start New Assessment.” Sign and complete your portion of the PHA.
- FOR MARINE AND NAVY: Once you have completed the online assessment, submit an appointment request via email to the distro: usn.pendleton.navhospcampenca.list.pha-imr@HEALTH.mil
- Please ensure that you include your DOD ID number and a good phone number.
- For army and Airforce complete part A then you will have to contact your respective bases and or go to fort Irwin to request an appointment.
