Welcome to your Pediatric Clinic Medical Home Blue and Green Teams located on the 2nd floor of Naval Hospital Camp Pendleton. We serve more than 9,400 enrolled TRICARE beneficiaries from newborns to 18 years of age.
Medical Home Port (MHP) is Navy medicine’s patient and family-centered medical home model. Your MHP team will ensure that care is all-inclusive and integrated with all other care provided within our healthcare system. Care delivered in MHP includes, but is not limited to: readiness, prevention, wellness, behavioral health, and disease management. MHP is intended to enhance the delivery of Quality Healthcare, putting our patients first.
New Patients: Please request medical records from your previous health care facility (unless patient was at a military treatment facility, as we will have the records in our system already) by calling the Medical Records Department at: 760 - 719 - 3137 or have the medical records faxed to the Medical Records Department at: 760 - 725 - 1547.

Forms/ Paperwork:
Generally, paperwork (such as school, sports, camp physical, health assessments), can be dropped off for provider to complete within 5 business days if there is a current physical on file.
