Welcome to your Family Medicine Department Medical Home Port.
We serve over 19,000 enrolled TRICARE beneficiaries (Active Duty, Retirees, and Family Members).
Medical Home Port (MHP) is Navy medicine's patient and family-centered medical home model. Under the Navy’s Medical Home Port Model, each patient is assigned to a team led by a provider. The patient is a part of the team that also includes a nurse, care coordinator, and other support staff. Your MHP team will ensure your care is all-inclusive and integrated with all other care provided within our healthcare system. The care delivered in MHP includes, but is not limited to:
· Well Child Exams
· Routine Obstetrical Care*
· Readiness
· Prevention
· Wellness
· Immunizations
· Vasectomies*
· Colposcopy*
· Osteopathic Manipulation*
· Behavioral Health*
· Acupuncture*
· Disease Management
*This type of appointment requires educational counseling and/or referral from the patient’s primary care manager in order to be seen in these specific areas of our clinic.
At the Family Medicine Department, patients are seen by appointment. Appointment times are based on severity of illness, the patient's convenience, and availability. Acutely ill patients will be seen within 24 hours, patients with routine problems will be seen within 7 days, and patients needing health maintenance will be seen within 30 days. A nurse is available to speak to our patients 24/7 through the Nurse Advice Line, at 1-800-TRICARE, Option 1.
