Exceptional Family Member Program (EFMP)
What is the EFMP?
The EFMP is a program offered by each service to support families with special medical and educational needs throughout the service member’s career. Family members should enroll in EFMP when a qualifying condition is diagnosed. Each branch has its own EFMP and enrollment process.
Families are Required to enroll in the Exceptional Family Member Program (EFMP) for two different reasons:
1. Document medical needs for potential enrollment into the Exceptional Family Member Program (EFMP), and or document early intervention / special education needs for potential enrollment into the Exceptional Family Member Program which supports military families with special medical and / or educational needs.
2. Document the potential medical and educational travel concerns of a family member during an Overseas Screening (OSS).
Forms
A) There is only one form necessary for enrollment into EFMP for medical reasons:

Who completes the form?
- The Sponsor, Parent or Guardian, or Person of Majority Age completes the demographics requested on the form.
- A Qualified Medical Provider is responsible for assessing whether the services they are eligible to prescribe are within the scope of their practice and their state licensing requirements. A Qualified Medical Provider may include a Medical Doctor (MD), Doctor of Osteopathic Medicine (DO), Physician Assistant (PA), Nurse Practitioner (NP), or Advanced Practice Nurse (APN).
B) There are two forms necessary for enrollment for educational / developmental needs:
- The Sponsor, Parent or Guardian, or Person of Majority Age completes the demographics requested on the form.
- A Qualified Medical Provider is responsible for assessing whether the services they are eligible to prescribe are within the scope of their practice and their state licensing requirements. A Qualified Medical Provider may include a Medical Doctor (MD), Doctor of Osteopathic Medicine (DO), Physician Assistant (PA), Nurse Practitioner (NP), or Advanced Practice Nurse (APN).
- The Sponsor, Spouse, Legal Guardian, or Student who has reached Age of Majority completes the demographics requested on the form.
- An early intervention (EI) or school representative.
3) In some cases, additional documentation such as an Individualized Education Plan (IEP),Individualized Family Service Plan (ISFP), or 504 Plan may be required. Those would be issued by the appropriate authority.
Next step
Once the required paperwork is filled out by the appropriate authority, it can be turned into the EFMP office (Patient Administration Office, NHCP First Floor). The EFMP coordinator will submit for processing. Once it is submitted, a case liaison will be assigned. An automatic email will be generated and sent to your listed government email address notifying you of your case liaison (this is not notification of enrollment).
The process takes approximately 6 - 8 weeks for enrollment to be completed. You should be sent a notification email once completed. If you have not received an email and it has been longer than 2 months, please contact your coordinator or case liaison.
NOTE: NHCP processes Navy dependents only. Other service dependents must work through their service offices.
